Stroke treatment: tele medical connection of family doctors and specialists

Mit dem Projekt INAN-SOS (Integration der ambulanten Nachsorge in das Schlaganfallnetzwerk Ostsachsen) wurde eine technische Verbindung zwischen der bereits eingesetzten technologischen Plattform THOS und den Systemen der Haus- und Fachärzte der ambulanten Nachbetreuung geschaffen.

The Stroke Network Eastern Saxony (SOS-NET) has developed beyond the Saxon region into a reference application for the ICT-supported treatment of acute stroke in inpatient care. Essential parts are an integrated clinical pathway (SOS-Care) which extends beyond rehabilitation for up to one year of the time of outpatient after care.

Since Mid of 2015, the technological base is built by the CCS Telehealth Ostsachsen platform (CCS-THOS), which was jointly developed by Carus Consilium Sachsen GmbH in cooperation with T-Systems International GmbH and which was supported by the European Union and the Free State of Saxony with ERDF Funds. This path is quality assured for the care and documentation technically implemented under connection of the acute and rehabilitation clinics. Within the framework of an aftercare program, the patient is support by one or more case managers by means of this pathway.

Currently, neither family doctors nor medical specialists or other professions who further attend to these patients (speech therapists, nursing care, physiotherapists) are not technically connected what leads to information and documentation loss.

To continue delivering important impetus with the reference application SOS-Care to strengthen and concentrating Saxon health care economy regarding its competence to realize innovative eHealth-solutions, the project INAN-SOS is supposed to close exactly this gap and to establish a technical connection between the currently used technological platform THOS and the systems of the family doctors and medical specialists of outpatient follow-up care.
